If you are worried about the safety or wellbeing of a child, young person, or adult, you can report your concern using the online form on this page.

This form can be used to report any safeguarding concern, whether or not it is related to Sunrise Sanctuary Healthcare services. You do not need proof or certainty to raise a concern, if something does not feel right, it is appropriate to report it.
